- Call Toll-free 1-877-757-1710 and give us your full name, the last four digits of your Social Security number, and your date of birth (our menu will prompt you through your choices), or
- Write to us attention privacy officer at First State Bancshares, Inc., 201 East Columbia St, Farmington, MO 63640, providing us your full name, the last four digits of your Social Security number, and your date of birth
Please note: If you are a new customer, we can begin sharing your information 30 days from the date we provided this notice. When you are no longer our customer, we continue to share your information as described in this notice. However, you can contact us at any time to limit our sharing.

Your choices will apply to everyone on your account.
Definitions
Affiliates
Companies related by common ownership or control. They can be financial and non-financial companies.
Our affiliates include:
- Financial Companies such as: First State Insurance Agency and First State Financial Management.
- Nonfinancial companies in which we will not share information.
Nonaffiliates
Companies not related by common ownership or control. They can be financial and non-financial companies.
- FSCB does not share with nonaffiliates so they can market to you.
Joint Marketing
A formal agreement between nonaffiliated financial companies that together market financial products or services to you.
- Our joint marketing partners include Credit Card Programs and our Accidental
Death & Dismemberment (AD&D) Partner.
